Advances in technology and information in the industrial world are Increasing rapidly, all activities are carried out online. Influenced by the Internet, especially websites are growing. Therefore a website is needed to mout the needs of companies in the field of technology that can help to make the company younger. Customer satisfaction is a major factor in the field of service or goods business. The company will get a big profit if the customer is satisfied with the optimal service provided by the company. Problems that occur in services and continue to occur with the same service system made the author take the initiative to create an application to evaluate customer satisfaction, the author provides a solution by using the Customer Satisfaction Application for Clean Water Waste. So the author's goal of making a Customer Satisfaction Application for Clean Water Waste is to determine the level of job satisfaction in CV. Mitra Mandiri Sejahtera towards customer service by creating a desktop web-based customer satisfaction survey application. The method uses the waterfall method and the database uses the MYSQL database, coding translates the system into HTML, CSS, PHP programming languages. The design includes Use Case Diagrams, Activity Diagrams, class diagrams.
